How to Complete Your PA Employment ApplicationIt is important that the following information is provided on your Pa personal statement as this will help to ensure that your application is completed properly. If the statement does not contain all of the information required, the application may be rejected. The applicant is responsible for making sure that all of the information requested in the Statement of Eligibility and Conditions of Eligibility (SCE) are provided. The basic information to include in the Personal Statement includes your social security number, name, date of birth, gender, and signature.For documentation purposes, a copy of the applicant's passport, if applicable, should be included on the personal statement. A current, valid photo identification and a copy of the applicant's most recent pay stub are required for some job positions. An applicant should check the employment requirements of the job they wish to apply for with their local human resources office to det ermine if the requirement includes any photo identification requirements.The Pa personal statement should always include accurate contact information. This information should be clearly stated on the statement and should be verified prior to submitting the Statement of Eligibility and Conditions of Eligibility (SCE). In order to complete the personal statement, the applicant should identify the hiring authority and the employee in the appropriate place. Applicant must enter accurate contact information when signing the application or forms. If you are asked to fax a copy of the certificate of naturalization, the applicant should verify that the document is received by the appropriate department of the issuing authority before submitting it.There are two types of acceptable documents that can be used as proof of citizenship: an original document issued by the US Department of State and a photocopy of a certificate of naturalization. The applicant should not submit originals of docume nts. If a photocopy is acceptable, the applicant should fill out the application with both copies of the document.The PA personal statement should contain the applicant's full legal name, date of birth, and address. Applicants must specify their correct last name or use initials only if this option is provided. While waiting for the paperwork to be received, the applicant should continue to apply for jobs in the local area. Although the SA and/or PA employment requirements vary by region, a job application submitt ed for each area will generally be accepted.Once all documentation has been received and forwarded to the issuing authority, the applicant should visit the regional office and request a copy of the PA employment eligibility form. This form must be sent with the applicant's resume and letter of intent. Instructions on how to complete this form will be included with the SCE or submitted through the I-9 form available on the USCIS website. Once the form has been received, the applicant should return it along with copies of their legal identification card and of their photocopies of documents as requested by the Office of Personnel Management (OPM).Once the applicant has submitted all requested documentation, they should call the Office of Personnel Management (OPM) to ensure that all required files are electronically submitted in accordance with the I-9 instructions. Any information contained on the application will be confirmed once the files have been submitted to the Office of Perso nnel Management (OPM).
